Inscription Information:
Mandatory items of inscription at Government expense are: Name; Branch of Service; and Year of Birth - Year of Death. Branches of Service are: U.S. Army, U. S. Navy, U.S. Air Force, U. S. Marine Corps and U. S. Coast Guard. Only one may be shown. NOTE: Army Air Corps and Army Air Forces were a part of the U. S. Army and, therefore, are not authorized for inscription at Government expense as Branch of Service.
OPTIONAL ITEMS which may be inscribed at Government expense are: Grade, Rate or Rank; War Service: Month and Day in the dates of birth and death; and an emblem reflective of a religious or non-religious belief.
MEMORIAL HEADSTONES AND MARKERS: The words "In Memory Of" are mandatory and will precede the authorized inscription data.
WHO IS ELIGIBLE?
Any deceased veteran of wartime or peacetime service who was discharged under conditions other than dishonorable. A copy of the deceased veteran's discharge certificate
(DD Form 214) is required.
